DECLARATION & RELEASE OF LIABILITY

DECLARATION: I, the applicant, declare the information I have provided on the application forms is correct, and all questions have been answered truthfully. I understand that UofN reserves the right to take necessary disciplinary action, including my being dismissed from the course/ school, if any information(s) provided by me is found to be untrue.

RELEASE OF LIABILITY: I, the Applicant do hereby release ASK / UofN, its staff, agents and volunteer assistants from any liability whatsoever arising out of an injury, damage or loss of property which may be sustained by said person during the course of involvement with the UofN Lonavala.

CONSENT FOR TREATMENT
 In case of emergency, I hereby agree to the performance of such treatment, including anesthesia and surgery, as the 
attending doctor or physician may deem necessary, at my cost.

ACKNOWLEDGEMENT OF FINANCIAL RESPONSIBILITY

 I understand that payment of the required school tuition fees must be made prior to or upon my arrival, unless otherwise 
approved in writing by the School Director / Leader before my arrival.  Further, I agree to meet in a timely manner, prior 
to the completion of School, all personal expenses incurred during my involvement with UofN Lonavala. 
If, I am accepted by UofN Lonavala, I will abide by the Spirit, rules and schedule of the school.

STATEMENT OF BURIAL & LIABILITY RELEASE

NOTE: Although death on the mission field is extremely rare, it is nevertheless an experience that awaits each one of us eventually. However, UofN Lonavala, does everything possible to protect its staff.

I agree that in case of my death while in UofN Lonavala, I hereby absolve all its staff and associates from any responsibility for burial costs. They may carry out the burial in the location of decease. If my family desires to see the body shipped home, my family will incur the cost.

In case of other loss of property, damage or personal injury, which may be sustained by said person(s) during involvement with UofN Lonavala, I/We do hereby release its staff, agents and voluntary assistants from any liability whatsoever.
